Are you unable to log into MnSP? Unsure if you have access to MnSP? Your mentor or security liaison can use the Support Plan User Access document to assist you. Users should not contact the MnCHOICES Help Desk or SSAM directly. Users should contact their mentor or security liaison.

Password resets: Go to your lead agency security liaison to request a password reset. Your password reset is good for 60-days. If your reset does not work, go back and request that your security liaison request another password. Note in the Hub Request that “this is a second request because the password is faulty”.

Before you report issues with logging into the Support Plan

If you are logging into MTZ-SP ensure you are using the correct user names. MTZ-SP User Names: Support Plan User Name example: Banana1, instead of Assessment User Name for MTZ-A: BAN001

If you are logging into the Support Plan Production remember you received two email messages from dhs.mnhelp@state.mn.us confirming your user name and password. The messages contain the subject line "Official MnCHOICES Support Plan New User Notification".

Password

If this is your first time logging in to the Support Plan, enter your temporary password. You will be prompted to change your password after you log in.

If you forget your password or your password is not working, have your mentor or supervisor contact the SSIS Help Desk to reset your password.

If you enter your password incorrectly five times in a row, your account will be locked, and you will not be able to log in. Contact your supervisor or mentor. They will contact the SSIS Help Desk on your behalf. When your account is unlocked, you will be given a temporary password, which you will be prompted to change after you log in.

Login button

Click the login button after you have selected your agency and entered your user name and password.

If you click the login button without filling in all three of the fields on the login screen, you will not see an error message which tells you which information you need to complete before you can log in.
