Persons with Serious and Persistent Mental Illness (SPMI)

Photo of powerless person sitting in chair in dark roomIdeally, a person who is receiving community services for SPMI (i.e. from Case Manager or other community support person) will, together with their support person, develop a Crisis Care or Crisis Prevention Plan.  If a Crisis Care Plan is on file with the Crisis Program, it will assist the recipient and the responder at times of crisis.  The recipient will not need to relate his/her entire mental health history and the responder will know what interventions are helpful for the individual.

A Crisis Care Plan includes:

  • Diagnosis
  • Treatment Providers/Programs
  • Medications
  • Baseline level of functioning and strengths
  • Chemical use
  • Triggers to increased symptoms
  • Early signs that person is moving toward a crisis
  • Coping behaviors and techniques
  • Supportive resources the person can call for assistance
  • History of danger to self or others

A psychiatric advance directive may include much of the same information and clarify the wishes of the individual.
