Current Nature of Functional Assessment
A functional assessment primarily reflects the person’s current functioning.
Functional assessment narratives should be updated frequently as the person’s abilities and functioning change as a result of services.
Minimally, the assessment should be reviewed and updated once every six months.
The description portion of a functional assessment can be conducted by the Mental Health Professional/Clinical Supervisor or a Mental Health Practitioner under the clinical supervision of a Mental Health Professional.
This description should include the person’s own descriptions of their functioning. This is a non-judgmental description; as it reports what is, rather than what should be.
