Quality Case Management

To provide quality Adult MH-TCM, the case manager must be skilled in the following areas:

  1. Identification and assessment of the needs, strengths and internal resources of the individual.
  2. Building a partnership with the individual to support and motivate the individual in his or her plan for recovery.
  3. Knowledge of local community resources and how to use those resources and the individual’s natural support system for the benefit of him or her.
  4. Building working alliances with representatives of these resources for the benefit of person served.
