Quality Case Management
To provide quality Adult MH-TCM, the case manager must be skilled in the following areas:
- Identification and assessment of the needs, strengths and internal resources of the individual.
- Building a partnership with the individual to support and motivate the individual in his or her plan for recovery.
- Knowledge of local community resources and how to use those resources and the individual’s natural support system for the benefit of him or her.
- Building working alliances with representatives of these resources for the benefit of person served.
