Assessment
Assessment means the case manager’s efforts with the individual to identify and discuss the individual’s mental illness, symptoms, functioning, vulnerabilities/risks, interests, culture, values, strengths, needs, skills, resources, support systems, goals, and concept of recovery.
Assessment also involves review of the individual’s diagnostic assessment, records of treatment, screenings, use of functional assessment tools, communication with past and present service providers, contact (with client permission) with the individual’s family and support network, and others involved in the individual’s life. However, most of assessment is done in conversation with the individual.
