This module provided you with insight into the various team roles and responsibilities. It’s based on a framework of person-centered planning and informed choice. An effective support plan assures that we are looking at what is most important to the person and for the person.

Each role is important in ensuring the person has the support needed to continue to maintain quality of life and function/independence.
