Young adults and adults with combined hearing and vision loss need access to supports that enable them to live independently.
Teens and young adults with combined hearing and vision loss need ongoing individualized supports that promote and prepare them for independence. These supports should reflect his or her preferences, interests and goals, unique situation and remaining hearing and vision.
Teens and young adults also need:
- To have their unique dreams and goals respected.
- Access to the information, supports and resources needed to achieve these dreams and goals.
- Access to trained educational and community Interveners, Orientation & Mobility services, etc.
- Opportunities to learn and use independent living skills.
- An Individualized Transition Plan (ITP) that outlines specific plans for transitioning from secondary school to adult life. This plan should include preparation for additional education, vocational training, future employment, and independent living. Transition plans must be incorporated in the child’s IEP by the time he or she is 14 years old.
- Access to transportation, community activities and appropriate supports.
- Access to meaningful employment and vocational training, as well as any employment supports needed to help the person to be successful.
- Access to trained Support Service Providers (SSPs) and other direct service providers who can help them live independently.
- Assistive technology and home modifications that increase independence and self-sufficiency.
- Access to safe, supportive living environments, including independent living options, group living options, etc.
