Housing Access Coordination Overview
People Supported by HAC
HAC helps adult Minnesotans who:
- Are enrolled on long-term care waivers.
- Want to live in a home of their own, that is not owned, operated or leased by a provider of service.
- Need help to find, move to, set up and then keep a home of their own.
- Agencies and staff must not have a direct or indirect financial interest in the property or housing the participant selects.

KEVIN
A Benton County caseworker referred Kevin to HAC when Kevin wanted to find a home of his own. Kevin receives the CADI waiver and had to leave an apartment. He ended up in a group home until another suitable living arrangement could be found. HAC met with Kevin and searched to locate an apartment. HAC also helped Kevin with rental deposit funds. Kevin's CADI service provider, Catholic Charities, assisted Kevin with furnishing his new home.
SEAN
Sean's mother contacted HAC because Sean had suddenly lost his roommate, so he could no longer afford his two-bedroom apartment. Neither Sean nor his mother thought returning to the family home was the best solution. HAC worked with Sean and his mother to discuss budgeting and seek an affordable new place to live. During the process, Sean said, "I really like my apartment and would like to stay since it's so close to everything I need." HAC staff worked with Sean's landlord and discovered that a one-bedroom unit was available. HAC staff worked with the landlord to reduce the rent to make it more affordable for Sean.
KATIE
A Benton county case worker contacted HAC inquiring about helping Katie move from an adult foster care home to her own apartment. Katie uses the CADI waiver program for her supports. HAC worked to locate possibilities for Katie and drove her to each potential home. She chose and was approved for a two-bedroom apartment with a roommate she selected. HAC also helped Katie with a security deposit and moving costs to assure the move was affordable. HAC staff stopped by after Katie moved in to follow up and share some information about community classes that would support her interests.
