Example 1: ADRIANNE


Adrianne

Before Adrianne called HAC, she didn't know what to do to find an apartment. She lived in an apartment for the last year leased by her PCA service provider, which did not allow her to control her services and home. When Adrianne's lease was up for renewal, the agency raised the rent to a rate that was unaffordable on Adrianne's fixed income. She called HAC and a housing coordinator responded immediately. HAC staff helped Adrianne find an affordable solution that had the modifications needed for her to function as independently as possible in a market rate apartment with supports through the CADI waiver.

The HAC worker coordinated Adrianne's Metro Mobility rides with apartment tours so Adrianne could choose an apartment with many of the features she was seeking. The apartment complex lacked certain features like door assists and door locks that Adrianne could operate easily. Work continues to modify the building so Adrianne's new home is as accessible as possible. During the person-centered budget planning process, Adrianne learned about the Minnesota Supplemental Aid Housing Assistance. HAC also provided money for the application fee, deposit and moving expenses. Adrianne was pleased with the assistance HAC provided in answering questions, helping to coordinate the move and negotiating the lease. Adrianne is looking forward to the spring when she can enjoy the gardens, lakes and vistas of her beautiful new apartment grounds and is planning for her daughter's visit so they can enjoy her new home together.

Example 2: MICHELLE

Michelle was connected with HAC by her caseworker in Clay County. She was interested in purchasing a mobile home, but the expense of moving the home was too great, so Michelle began working with HAC to find an affordable place of her own. Michelle was ready to move and use her supports from the CADI waiver program in her own place. After pursuing several possibilities, HAC staff was able to help Michelle locate an apartment that would also accept her dog. HAC helped with the application fee, security deposit and paperwork needed for approval of the apartment. Staff worked with Michelle to locate a bed for her new place via their relationship with The Arc thrift store. When Michelle moved in she said, "I am thrilled to be in this beautiful apartment with so much space and Lady (my dog) can stay with me."
