The Deaf and Hard of Hearing Services Division has developed a set of supplemental questions designed to highlight some of the unique aspects of deafblindness. This information is critical to creating an effective support plan for an individual with combined hearing and vision loss.
The Supplemental Questions augment seven key categories of the Minnesota Long-Term Care Consultant (LTCC) questionnaire:
- Independent Living: Instrumental Activities of Daily Living (ADLs)
- Caregiver Supports/Social Resources
- Health Assessment
- Functional Assessment/Activities of Daily Living (ADLs)
- Emotional and Mental Health
- Assessment Results
- Caregiver Assessment
