Caseworker with clientHow and where are MH-TCM services provided?

AMH-TCM services (in Minnesota) assist individuals with “serious and persistent mental illness” (SPMI) to obtain services and supports to optimize their participation in the community, to assist in accomplishment of their goals, and to address assessed needs. 

Case management services are usually provided in the client’s home and community and site of referral for needed services/resources/support as opposed to an office/clinical setting. 

The case manager can coordinate and monitor the services that the individual is receiving from resources (for examples) for: housing, financial benefits, health care coverage, physical health care, transportation, employment, education, mental health, socialization, recreation, spiritual interests, household skills and management, activities of daily living, and other needs/goals.
