The following are examples of Targeted Case Management Activities:
- Assisting a consumer in obtaining needed services.
- Coordinating with other agencies to obtain services for consumers.
- Completing and regular review of a written functional assessment.
- Developing and regular review of a written individual community support plan with the client that is based on both a functional and a diagnostic assessment and incorporating the individual treatment plans.
- Participating in case conferences, administrative reviews, client staffing and informal conferences.
- Developing goals, service plans, written service agreement and routine case supervisory activities.
- Contacting on a routine basis and other monitoring or communication with the consumer, family members, or other relevant persons regarding the client’s status, the individual service plan or progress in achieving the goals of the service plan.
- Receiving clinical supervision regarding an identified client.
- All planning, assessments, record keeping and documentation associated with the service coordination of a case.
- Assessing and evaluating the effectiveness of services received by a consumer.
- Arranging for transportation, lodging, meals and/or child care needed for a consumer to access services.
- Assisting the client in obtaining third party payment for covered medical services.
- Monitoring the discharge planning process for a client being discharged from an intensive residential treatment setting, regional treatment center, community behavioral health hospital or a community hospital inpatient psychiatric unit.
