Meet with County Case Managers

HAC staff develop person-centered housing plans by working with individuals, supporters and case managers. Your relationship with county and tribal case managers is essential to the HAC process.

Meet with county and tribal case managers to:

Arrange for service planning changes.
Obtain approval of HAC services for waiver participants.
Plan, approve and arrange for other program supports such as:
- Transitional services
- Individual home-based supports such as Independent Living Services (ILS) or Personal Care Assistance (PCA)
