Who is Eligible for HAC?

People eligible for Housing Access Coordination (HAC) are adult Minnesotans of all ages who:

  • Are enrolled on Home and community based disability long-term care waivers. • Want to move to homes of their own that are not provider leased, ownded or controlled.
  • Who want to move to homes of their own.

Confirm documentation to verify HAC eligibility:

  • Obtain the notice from the MN DHS sent to the person's county or tribe of financial responsibility, or
  • Obtain the form signed with the appropriate eligibility determination by the person's county or tribe representative.
  • If the person has not been assessed to access home and community based waiver services, help the person contact the local human services agency to determine eligibility.
