Housing Access Coordination Process

The Plan stage of the HAC process is critical as it helps lay the groundwork for success and determines what that success looks like.

Your first action in the Plan stage is to meet with participants to develop their person-centered plan. The essential planning task calls you to work with individuals to:

Clarify their goals, hopes and needs.
Identify potential supports.
Create the budget and housing checklist.

The person-centered plan forms the basis for the type of housing the person seeks.
