Begin Date Factors

Use the following factors to help you determine the begin date for MA-LTC:

The date the request was received at your county.

The date of admission
Use the Physician Certification form (DHS-1503) to determine the date of admission or the begin date of skilled nursing facility services if received in a hospital. More information regarding the DHS-1503 is discussed later in this course.
for a client residing in an LTCF.

The date of assessment as determined by the LTCC
This requires a Lead Agency Case Manager/Worker Communication Form (DHS-5181) on file that indicates the LTCC has been completed that resulted in a determination that the client needs waiver services and meets LOC requirements to begin those services. More information about the LTCC is discussed later in this course.
for clients requesting services through one of the home and community-based waiver programs.

Retroactive months requested.

Clients residing in an LTCF may have MA-LTC eligibility determined back to the basic MA eligibility begin date or when the client entered the LTCF, whichever is later. For clients requesting services through one of the home and community-based waiver programs, MA-LTC cannot begin prior to the LTCC date.
