Additional Reasons for LTCC and Full Team DD Screenings

The purpose of the long-term care consultation (LTCC) and the full team DD screening also includes the following:

• Informs clients about the availability of community support services, including Minnesota Health Care Programs (MHCP).

• Assesses a person’s health, psychological and social needs to identify the type of services the person needs.

• Provides information about community-based options as an alternative to or a transition from nursing facility placements. These options are less restrictive and may be more cost-effective than an LTCF placement.

• Develops a community support plan for the person.

• Recommends a nursing or boarding care facility placement when no cost-effective community services are available.
