Request for MA-LTC through a Waiver Program

You will know a person is requesting MA-LTC through one of the home and community-based waiver programs if you receive one of the following and the person is not residing in an LTCF:

• Minnesota Health Care Programs Application for Payment of Long-Term Care Services (DHS-3531).

• Minnesota Health Care Programs Request for Payment of Long-Term Care Services (DHS-3543).
