Make a referral to the county LTCC team, even though Sally did not request MA-LTC, because Sally may benefit from home and community-based services. MA-LTC eligibility cannot be determined until all LTC requirements are met, including the client's request for MA payment of LTC services and proof that client meets an institutional level of care via the DHS-5181.
