Use Emily’s case information to answer the following questions.

1. What is Emily’s MA basis of eligibility?

Age 65 and over (EX). Her date of birth is listed on her DHS-3531 as December 05, CY-89.

2. Has Emily provided a Social Security number (SSN)?

Yes, she has provided an SSN. The SSN will be verified through a data exchange with the SSA.

3. Has Emily provided proof of her citizenship and identity?

No, however, Emily is exempt from this requirement due to her receipt of Medicare. You will be completing the required case note in the MA-LTC Systems classroom course for Emily to record her exemption from this requirement due to receipt of Medicare.

4. Does Emily meet Minnesota residency requirements?

Yes, Emily lives in Minnesota and intends to remain in the state while residing at Bethany Covenant Home.

Did you notice that Emily’s home is in Ramsey County but she is residing in an LTCF that is in Hennepin County? Ramsey County is the county of financial responsibility because that is the county in which Emily resided prior to entering the hospital and the LTCF. Both the hospital and the LTCF are excluded time facilities. Hennepin County is the county which will service Emily’s case. More information about the county of residence and servicing county issues will be provided in the LTC Systems course.
