2. Has Padma provided a Social Security number (SSN)?

Yes, Padma has provided an SSN. The SSN will be verified through a data exchange with the SSA.

3. Has Padma provided proof of her citizenship and identity?

No, however, Padma is exempt from this requirement due to her receipt of Medicare. You will be completing the required case note in the MA-LTC Systems classroom course for Padma to record her exemption from this requirement due to receipt of Medicare.

4. Does Padma meet Minnesota residency requirements?

Yes, Padma lives in Minnesota and intends to remain in the state while residing at home in St. Paul.
