Disabled. Even though Isabella is under the age of 21 and could use a child basis of eligibility, people on a disability waiver must use a disability basis of eligibility. Isabella’s disability was verified by SMRT.

2. Has Isabella provided a Social Security number (SSN)?

Yes, she has provided an SSN. The SSN will be verified through a data exchange with the SSA.

3. Has Isabella provided proof of her citizenship and identity?

Yes, a copy of Isabella’s passport has been provided. You will be entering the required case note in the MA-LTC Systems classroom course to record the type of verification received.

4. Does Isabella meet Minnesota residency requirements?

Yes, Isabella lives in Minnesota and intends to remain in the state while living in St. Paul.
