Expectation to Return Home
To determine if a client is reasonably expected to return to her home, review the Physician Certification Statement (DHS-1503) that you will receive from the LTCF. The form will indicate the expected length of the client’s stay and health condition.
- If the form indicates the client is expected to be discharged from the LTCF and does not have any health conditions that would require the client to have extra help upon his return home, document that in MAXIS case notes and allow the exclusion.
- If the form indicates the client is not expected to be discharged from the LTCF, request a doctor statement indicating when the client can reasonably expect to return home. Allow the exclusion if the doctor statement indicates the client is expected to return home. Document this in MAXIS case notes.
- If the form indicates the client’s heath or condition will require at-home help in order to return home, document how that help will be paid for. MA, services provided by a waiver program, Medicare or other health insurance are acceptable documentation. Document this in MAXIS case notes.
