Use Isabella’s case information to answer the following questions.

1. Does Isabella have other health care coverage?

Yes, based on the information provided on the DHS-3531, Isabella has Blue Cross Blue Shield coverage through her father’s employer.

2. Is Isabella’s other health care coverage cost-effective?

Yes, based on the Request for Employee’s Group Health Plan Information (DHS-3767A) and the Determination of Cost Effectiveness (DHS-3767D), Isabella’s coverage is cost-effective. This determination was done prior to application processing and the documents have been filed in her case file. We will reimburse Isabella’s father for Isabella’s portion of the premium, which cannot be used as a health care expense.

3. Does Isabella have any other third party liability issues?

No. No other health care coverage or accidents were reported on Isabella’s DHS-3531.

4. Does Isabella have to cooperate with medical support or have a parental fee assessed?

Yes, a parental fee will need to be assessed because Isabella’s parents’ income is not considered in determining Isabella’s eligibility. A medical support referral is not needed because both of Isabella’s parents live with her.
