Introduction
The service delivery is documented in service progress notes. Notes should link back to service plan and ultimately the functional assessment. Progress notes describe the actual goals and objectives the intervention is designed to achieve.
These notes provide a detailed description of the staff’s intervention(s), the consumer’s response(s) to the intervention and the provider’s plan for next contact. Each progress note is a mini-reassessment and the culmination of progress notes over time.
