Purpose of FA
The functional assessment data can be used to identify how the consumer’s symptoms related to how they experience their mental illness and how that impacts their ongoing status and functioning in particular areas of their life.
The functional assessment helps support program eligibility into case management services by identifying those domains in which the consumer has impairments in status and/or functioning. This aids case management providers in prioritizing, monitoring needs and identifying referral, linkage and coordination.
