Diagnostic Assessment (DA) review by case manager
Diagnostic assessment, Minnesota Statute 245.462, Subd. 9., means a written summary of the history, diagnosis, strengths, vulnerabilities and general service needs of an adult with a mental illness using diagnostic, interview and other relevant mental health techniques provided by a mental health professional used in developing an individual treatment plan or individual community support plan.
Ideally, the DA supports the functional assessment and ICSP by:
- Explaining why the mental illness diagnosis is affecting the individual in specfic ways – symptoms and functional limitations;
- Points to areas of functional impairment and needs that the ICSP might address in planning and referral; and
- Establishes who this person is – not just the illness – preferences, strengths, and history.
