The Individual Community Support Plan (ICSP)
The second core service component of adult mental health targeted case management is planning – the development of the care plan with the client. In Minnesota Statute, the care plan is called the Individual Community Support Plan (ICSP).
The ICSP is defined as a written plan developed by the client and case manager on the basis of a diagnostic assessment and functional assessment. The plan identifies the client’s goals, and specific services and resources needed by the client to develop independence and improved functioning. The ICSP identifies client goals, timeframes, specific tasks and implementation activities, and responsibilities of the client, the case manager and others involved.
The development of the ICSP should be in partnership with the client, and based on person-centered principles.
