Functional Assessment (FA)
It is not sufficient to complete checklists or numerical rankings of a client’s functioning domain areas. Written narratives in each of the domain areas of function of the individual recipient must be document and reassessed in written narrative.
Minnesota adult MH-TCM case managers are required to complete a functional assessment of a client and his or her service and resource needs within 30 days of MH-TCM services beginning and at least within 180 days thereafter.
Best practice is that the functional assessment and the ICSP are reviewed, updated and completed with the client every 90 days. This may happen more frequently upon client request or if there are any significant changes in the functioning or needs of the client or if progress toward the outcomes planned in the ICSP are not occurring. The case manager should not wait for 90 or 180 days if the ICSP is failing.
