Case managers are uniquely positioned to help integrate health care services received by clients. Not alone, but in a partnership role.
Case managers can do basic screening and assessment of health problems, diseases and lifestyles.
Case managers can assess and monitor client access to basic health services (e.g. physicals, well-child visits, blood tests, dental check ups).
Case managers can assess what health care treatment has been prescribed for a client’s health problems, and encourage clients to follow recommended care for physical and mental health concerns.
Case managers can coordinate with health care providers.
Case managers can support referral and linkage to recommended health care services.
Case managers can obtain more training specific to health assessment.
Assist recipients in maintaining their health care/insurance coverage.
