The ICSP must include:

  • The participation of client and goals of the client, preferably in the client’s own words. The goals and objectives should be measurable.
  • Services, resources and supports to be acquired are identified;
  • The tasks and action steps to be taken by the case manager to support the acquisition of resources, supports and services by the client to accomplish each goal;
  • The tasks or action steps to be taken by the client and others to acquire resources, supports and services to accomplish his or her goals
  • The schedule of each task and action
  • The frequency of face-to-face contacts by the case manager and client;
  • The incorporation of the recipient’s treatment plan’s into the ICSP;
  • To the extent possible, the involvement of the client’s family, advocates, providers and others;
  • The signatures of the client, the case manager; and the approval and/or signature of the clinical supervisor and
The distribution of the ICSP to the client, the case manager, and involved parties (with releases).
