Case Notes
As the other health care program courses emphasize, case notes are essential to effective case management. Always document all case information and actions in case notes.
There may be times when you must enter a specific case note for some MA-EPD cases as explained below.
A DHS report will inform workers of the number of months an enrollee received MA-EPD the month before they turn age 65. Enter a Y or N on MAXIS STAT/PDED and enter the following case note:
***(enrollee’s name) was enrolled in MA-EPD for (# of months) prior to turning age 65.***
Enter the following case note when:
***This enrollee has received MA-EPD for (either 20 of the last 24 months or 24 consecutive months) before turning age 65. The following assets will be disregarded while this enrollee remains eligible for basic MA. (list assets) (If applicable) The enrollee’s spouse’s income will be disregarded while this enrollee remains eligible for basic MA. ***
Enter the following case note when an enrollee turns age 65 and is eligible for MA-LTC through a waiver:
***Legislation effective 4/1/12 allows this enrollee to remain on MA-EPD past age 65.***
