Functional Assessment over Status, Functional Description, and Link to Mental IllnessA functional assessment should contain the following elements:

  1. Status
  2. Functional Description
  3. Link to Mental Illness if there is one

Both the status and functional description require important data to be gathered, assessed and documented in the functional assessment.
