Functional Assessment over Status, Functional Description, and Link to Mental IllnessThe assessor has to consider numerous resources when collecting data to formulate a robust and thorough functional assessment.

Along with the primary sources of direct input from the consumer and the assessor’s observations and knowledge of the consumer, the assessor should include as many sources and methods as possible to develop the status, functional description and link to the mental illness.

Tools and techniques include asking the consumer the following types of question related to each domain:

  • “Are there times you can do this better than others?”
  • “How are you able to do this?”
  • “How well?”
  • “Show me how you would?”
  • “What helps you do this?”
  • “When can you do this?”
