Data Resources and Forms

Functional Assessment over Status, Functional Description, and Link to Mental IllnessIn addition to consumer input (verbal and demonstration) and assessor observation, other data sources should be considered.
These include:

  • Collaterals.  Collaterals can include family, friends, peers and people in the community who know the consumer. If the consumer gives their permission, these resources are a good source of information regarding status and functioning.
  • Professional resources.  Professional resources include reports and documentation from current treatment providers.  It is important that these resources be current.
  • Other formal and informal assessment data. For example, the diagnostic assessment data is essential for making the link to mental illness once status and functional description have been established.
