Functional Assessment over Status, Functional Description, and Link to Mental IllnessThe Minnesota Adult Mental Health Division does not require any form or specified format for conducting or documenting the functional assessment. Agencies are free to create their own method and format for documenting the functional assessment process provided that method and format met certain requirements.

These requirements include:

  • All required domains as defined by the state
  • Status and full functional description (inclusive of strengths of function and impairment) of all domains
  • Link to the consumer’s mental illness if there is one
  • All domains and required content are assessed each and every time
  • The term “no problem” is not used
  • The term “no change” may be used in a very limited measure for functional assessment updates
  • The actual functional description is expected to change upon update for any domain on which there are focused interventions provided
