Personal Futures Planning

Often during PFP, one area emerges as the priority for development. For example, Lila's plan reflects an emphasis on home, an alternative to the nursing facility that had been her only choice of residence in the past.

Your role with Lila might be to:

• Ask questions that reveal Lila's housing goals, hopes and needs.
• Discuss options for meeting Lila's housing priorities.
• Work with Lila to develop action plans for seeking and locating housing to meet her vision.
• Follow up with Lila as her goals, hopes and needs evolve.
