- Relocating to the community from an institution or mental health program, or
- Eligible for state plan services or a home and community-based waiver.

HAC staff help people access MSA Housing Assistance by:
- Informing potentially eligible people.
- Assisting individuals and/or their Circle of Support representatives to contact their county or tribe of financial responsibility.
- Performing other necessary tasks to ensure access to housing assistance.
- Completing DHS Form 5986: Verification of Closed Housing Authority Waiting List for Housing Assistance when the housing authority waiting lists are closed.
Eligibility
To be eligible for housing assistance benefits a person must be:
- Eligible for MSA.
- Have shelter costs totaling more than 40% of his or her gross income.
- Under age 65.
- Relocating to the community from an eligible institution or be enrolled on PCA services or a home and community-based waiver.
- Determined by the county to be eligible for housing assistance.
- Applying for a subsidized housing program. (If the person is currently receiving subsidized housing; he or she would not be eligible for housing assistance.)
Guidelines
- Participants receive benefits monthly via direct deposit to a financial organization, e.g., bank, credit union, or trust, or to a representative payee.
- The amount paid in by the insured worker determines the benefit amount.
- People apply through local Social Security Administration offices.
