Housing Access Coordination Process

You learned in the first module of this course that Housing Access Coordination (HAC) began as a demonstration project. One important outcome of the original project is a proven process that produces successful housing results. This module outlines the HAC process.

Before you get started with the HAC process, make sure you know how to:

• Implement person-centered principles as you interact with every person (Module 2).
• Use person-centered planning methods to clarify the person's hopes, dreams and goals (Module 2).
• Carry out your HAC responsibilities (Module 3).
• Verify eligibility and discuss funding to get a clear picture of the person's resources (Module 4).
