In general, people make initial contact with HAC due to:

  • A county referral, e.g., case manager.
  • Word of mouth.
  • Contact with promotional tools like a flyer or brochure.

Your first point of contact with participants involves a discussion to verify eligibility. After conducting the initial conversation, schedule a meeting to develop the person-centered housing plan. During the first conversation, the Housing Access Coordination process begins.
