Communicate with the County

Once individuals receive their copy of the executed lease, they may need your help to arrange funding and services.

Communicate with the person's county of residence to:

• Provide notification of the impending move.
• Obtain the transitional waiver services for goods and moving expenses

In addition you may need to:

• Schedule moving and packing services
• Help getting household goods
• Arrange any neccessary deposits
• Schedule an Occupational Therapy (OT) assesment in the home for modifications or assistive technology that may be needed prior to the move.
