MA-LTC Forms
As previously mentioned, not all DHS applications and renewals request the additional information needed to determine eligibility for MA-LTC. Because of this, specific forms for MA-LTC eligibility have been created.
The appropriate form to use is based on when the person is requesting the payment of long-term care services. Regardless of what application or request form is used, you may still be required to follow-up with the client to be sure that you have all of the required information needed to determine eligibility for MA-LTC.
