While it is not required that the assessor separate out the status and functional description into separate distinct boxes for each domain, it is required that each domain contain both status and functioning.
On the next screen are examples of the status and the functional description for the social and vocational domains. Please note that functional descriptions include both impairment and strength of functioning.
