Real world measures

As discussed previously, goals and objectives must be observable.
Specificity of the measurement will depend on a number of factors including the service setting, level of care, opportunities for observation, risk and safety issues, the ability of the person to self-report and the objective.

What is critical is that no objective measure should ever be written in such a way that the person is guaranteed to fail. It is required that you include the method by which you and the individual will measure progress (target objective), the frequency and specific times in which you will look at “data” and what progress you are expecting along the way.

Objectives require baseline and target measurements.

  • Target measurements are typically incorporated into the written objectives.
  • Baseline measurements are often written separately, but can be incorporated into the written objectives.
