Introduction
Welcome to Module 8, Goals and Objectives. Goals and objectives are one of the critical components of an individual’s Individual Treatment Plan (ITP). Goals and objectives are derived from information collected during the assessment process. Information regarding their mental health and how it impacts the recipient is collected from the recipient, invited family members and other staff involved with the recipients treatment.
Well written SMART goals and SMART objectives inform the individual and the provider of the types of services, intensity of services and progress indicators designed to help the individual achieve his/her designated goals and objectives. Goals and objectives demonstrate compliance to required state and federal regulations by providing a written, legal record reflecting the linkage between the course of treatment, services delivered and progress made.
Goals and objectives use person centered language that is clear, concise and easily understood. Specific interventions (what the mental health worker will do) are documented on the ITP for each objective, including the intensity, frequency and duration to achieve desired objectives.
