Four Core Components: Assessment and Planning
Assessment: The process of gathering information about the client’s strengths, needs, history, aspirations, culture, resources, mental illness, health practices, lifestyle through interviews, review of the diagnostic assessment, review of past treatment and other documentation, completion of screenings, use of assessment tools, observations, completion of the functional assessment and level of service needs determination.
The assessment includes the authorized input from members of the client’s formal and informal support network.
Planning: The development of priorities, goals, resources/services/supports to be accessed and the tasks/activities and timelines and responsibilities into the Individual Community Support Plan (ICSP).
The individual should be involved as much as possible in the assessment and development of the ICSP.
