Role of the case manager in planning
The role of the case manager with the client in planning is to:
- Assist the client to prioritize his or her needs, aspirations and goals;
- Establish goal statements based on the assessment and the recipient’s words (if possible);
- Design a plan that will support the client’s progress toward the goals;
- Identify existing support, resources, supports and services that will help the client to accomplish his or her goals;
- Define the necessary tasks and actions steps to access natural supports and services and resources to accomplish the goals, timelines and responsibilities and
- Involve the clinical supervisor in consultation about and approval of the ICSP.
