Clients, like many of us, may not be ready to change and set goals. Recipients may be ambivalent or need more time to process and think about committing to new goals.

 The case manager may have to help the recipient through the stages of change described by DiClemente, 2007:

  • Precontemplation
  • Contemplation
  • Preparation/determination
  • Action
  • Maintenance/Retention and
  • Re-evaluation.
Planning is an ongoing process that drives the development and periodic revision of an ICSP based on the continued information collected from the client, family, and other personal supports; from assessments or reassessments; and monitoring of client progress toward goals and the effectiveness of resources, supports and services.
