The ICSP must be developed within 30 days of client intake and rewritten or updated at least every 180 days after the ICSP is developed. The ICSP needs to be reviewed every 90 days if requested by the recipient; or updated if significant change in the priority needs of the recipient or if monitoring of the plan indicates that the plan is ineffective. The case manager does not wait 6 months to change the plan; if it is evident that parts of the plan/services received by client are ineffective, change the plan.

DHS recommends that Functional Assessment (FA) and ICSP be updated at least every 90 days, or more often if significant changes in goals, tasks, resources to be used, or obvious lack of progress with current plan.

Although the standards for Minnesota’s MH-TCM require that a FA and ICSP be completed within the first 30 days of case management services, it is okay to initially establish an ICSP with a single goal. The FA process is ongoing and additional goals can be added at any time. Work with the client; let the client set the pace as much as reasonable.

In general, the client should assume as much responsibility as possible for goal action tasks. Early on this should be defined and discussed between the client and case manager concerning what case management services are and the role of the case manager.
