DHS-5990 - MA-LTC Case Checklist
The MA-LTC Case Checklist is a three-page checklist you may choose to use when processing an MA-LTC case. It is not a required form when you process cases in your actual caseload.
The checklist provides you with:
- A way to remember to apply all of the different eligibility criteria for both MA and MA payment of LTC services.
- Some reminders of the details to check for when looking at those eligibility criteria.
- A way to track what verifications are needed and which have been received.
You will be completing part of the checklist as you review each of your MA-LTC cases in your training caseload for the policies provided in this course. These checklists will be used again in other courses in the LTC/Waiver course sequencing and will ultimately be used in the MA-LTC Systems course.
